Sonnet Home Health, a Medicare Certified Home Health Agency, is seeking RNs, LVNs and Therapists to provide high quality skilled care for our patients.
We are recruiting for our growing Orange County market and beyond. We are primarily seeking Per Diem Full Time or Per Diem Part Time.Minimum Qualifications:
Registered Nurse Case Manager -- At least 1 year Home Health experience, Excellent Assessment, Communication and Documentation skills, OASIS-C Knowledge
Licensed Vocation Nurse -- At least 1 year Home Health experience, Excellent Clinical Skills
Therapists -- PT/OT/ST -- At least 1 year Home Health experience, OASIS-C Knowledge strongly preferred
All positions require focus on Customer Service, Patient Care and Being a Team Player.
