LVN Case Manager (Skid Row)

Lamp Community ends homelessness of Los Angeles' most vulnerable individuals, primarily adults with mental illness, through a continuum of services and housing, enabling them to reach their highest level of self-sufficiency and community integration.
LVN Case Manager--Full-time
The LVN Advocate will work with members to coordinate mental health and recovery services and help them develop a plan to assist them in maintaining improved physical and mental health.
ESSENTIAL DUTIES & RESPONSIBILITIES
LVN-Related
1. Provide members with medication management including administration of medications; coordinate and assist with various appointments, and provide wellness education, along with referral and linkage to medical services.
2. Conduct weekly exercise classes for members; facilitate member health group meetings
3. Attend project trainings on chronic illnesses, nutrition, and benefits enrollment
4. Participate in project meetings.
5. Provide Case Management; coordinate, and implement mental health and recovery support services with individual members
6. Develop effective, trusting relationships with members, with a focus on facilitating independence and maintenance of improved physical and mental health
7. Assist in the screening, assessment, and enrollment of members, including orientation to program benefits, policies, resources, and goals
8. Develop and review an individual service plan for each member to be reviewed and updated quarterly
9. Assist with program evaluation and modification
10. Maintain a current, thorough knowledge of community resources and utilize these to provide comprehensive, wrap-around services to members
11. Assist in involving members in daily activities and scheduled appointments, budgeting, facilitating socialization, health maintenance, sense of community, and progress in recovery
12. Work cooperatively and cohesively with other members of the staff team, including participation in weekly staff meetings and staff trainings
Reporting
1. Develop and maintain a complete, accurate, and current member file with all required documents
2. Learn and utilize Exym database software
3. Maintain up to date, accurate, and complete data and progress notes as required by the project and its funding sources
4. Produce monthly and quarterly reports
5. Assure all member paperwork and data collections is complete and up to date in the information systems
QUALIFICATIONS
1. Proof of graduation from an approved nursing program with an associate's degree or equivalent
2. California license as a Licensed Vocational Nurse
3. Minimum two years' supervised experience as an LPN/LVN within the last four years.
4. Able to develop patient care programs within the clinical policies and program objectives and relating them to the funding requirements
5. Able to demonstrate sensitivity to the needs of members
6. Current, valid California Driver's License with an acceptable driving record
7. Ability to work with minimum supervision
8. Detail oriented with excellent time management, organizational, written, verbal, and computer skills
9. Minimum 30 WPM typing speed preferred
10. Experience working with homeless individuals, preferably those struggling with mental illness and / or substance addictions preferred
11. Bilingual in English/Spanish preferred
