Per Diem Care Manager (LVN or RN) (Riverside / San Bernardino)

ACCSS Inc is currently serving members of an insurance plan who have care management needs. These members range in age from 5-80 years old and live in Riverside, San Bernardino, Los Angeles and Orange Counties.
This opportunity is per-diem/Indepdent Contractor position; but can easily turn into a "near part-time or full-time" position (in caseload) for those who are motivated and successful. A "bonus" structure, based on quality not quantity work, is in place to acknowledge those who provide high quality services. The work is "field-based", meaning you will be conducting health evaluations and care management services for members in their homes and communities.
ACCSS Inc is seeking to expand it team by contracting with additional nurses (LVN or RN) who:
1) Have a license in good standing
2) Have had prior experience providing care management or community-based nursing services
3) Has professional liability insurance coverage (or willing to obtain)
4) Willing to submit to an extensive background check
The ideal candidate is someone who is:
1) Responsible
2) Self-directed; self-motivated
3) Sincerely interested in serving the members/patients
