Qualifications
Must be a graduate of an accredited school of nursing, RN or LVN. Prefer prior experience with senior adults and knowledge of regulations pertaining to assisted living communities.
Must be able to read, write, and speak the English language.
Must be able to communicate effectively with residents, families, physicians, and associates.
Physical Demands
Requires moderately heavy physical exertion on a regular and recurring basis. Regularly required to use hands to finger, handle, or feel. Frequently required to stand, walk, reach with hands and arms, sit, stoop, kneel, crouch, crawl, talk, and
hear. Occasionally required to lift and/or move up to fifty pounds. Specific vision abilities include close, peripheral, depth perception, and ability to focus.
Working Conditions
Assisted living community and general office environment. Potential for direct exposure to blood, body fluids, infectious diseases, air contaminants, and hazardous chemicals.
Essential Functions
a. Assist the Director of Resident Services in providing case management services to residents, ensuring completion of the pre-occupancy screening, assessment/reassessment, and the service plan in conjunction with the resident's family and other staff.
b. Facilitate and encourage family involvement with residents.
c. Assist in coordination of resident move-in and orientation of new residents to the community. Assist in the coordination of move-out, ensuring a smooth transition for the resident and family.
d. Investigate resident and associate incidents, identify safety hazards, and institute corrective actions in a timely manner.
e. Monitor resident service plans, record information to determine quality of service to residents, and direct staff to implement needed changes.
f. Monitor and maintain records reflecting resident's physical and mental condition. Report significant changes in condition to the Executive Director and physician.
g. Coordinate wellness activities and participate in the resident activity program.
h. Participate in implementing the Community Marketing Plan.
i. Ensure compliance with regulatory agencies. Participate in facility surveys initiated by government agencies and/or management.
j. Work with other members of the management team to market and manage the community.
k. Perform all other duties as assigned.
