Seeking LPN Case Manager (Miami, FL)

Clinical Liaison
As a member of our team, a clinical liaison will generate patient admissions, from hospital based referral sources and promote the growth of admissions over time . Establish and maintain significant professional relationships with physicians and hospital personnel. Explain the services we provide to patients and families in a manner that results in the admission. Interact with patients, families, and referral sources. Oversee patients and routinely follow the progress of prospective patients. Work closely with our teams in transitioning patients as they are being admitted. Travel as necessary.

Ideal Candidate will have the following qualifications:
Â• Minimum age requirement of 18
Case Management Experience
Licensed Practical Nurse required and must be license in State of Florida
Â• Minimum of three years general nursing, medical, surgical or critical care experience preferred.
Â• Ability to market to and deal tactfully with customers/patients referral sources and the community.
Â• Excellent communication, negotiation and public relations skills.
Â• Excellent presentation and public speaking skills.
Â• Compliance with accepted professional standards and practices.
Â• Valid driver's license with maintenance of safe driving record and an automobile that is insured in accordance with State of Florida
Â• Ability to work in an interdisciplinary setting.
Must be a Team player
Â• Self-directed with the ability to work with little supervision.
Â• Flexible and cooperative in fulfilling all obligations.
Spanish speaker preferred
