Job Responsibilities: * Responsible for providing member primary care sites with the sampling and medical record abstraction component related to NCQA Diabetes Recognition Program, as well as providing supplemental reinforcement on sampling techniques, electronic data options and NCQA DPR requirements

Job Requirements:
Required Experience: * Completion of LPN program required * Valid license in New York State * Experience working in Medical Record Abstraction * Ability to be on feet and mobile during entire work shift
