We are committed to serving all of our client’s needs and providing superior care, and that all begins with wonderful caregivers.LPNProvide skilled professional nursing care to patient/family as directed by the supervising Case Manager or the Patient Care Manager and in compliance with all applicable laws and regulations and with NBD’s policies and procedures. Provide timely documentation of patient services based on plan of care. Identify other physical, psychological, social and spiritual needs for assigned patients. Work closely with other members of the interdisciplinary team to ensure all patient and family care needs are being met.Responsibilities include:• Utilizes the nursing process to assess, plan, implement and evaluate patient care.• Assess signs and symptoms indicating physiologic and psychosocial changes in patient’s condition.• Collects, analyzes, and interprets data and information from health care members and documents actual and/or potential nursing diagnoses.• Documents the patient’s plan of care using identified nursing diagnoses, expected patient outcomes, and selected nursing interventions.• Identifies and prioritizes actions based upon patient care requirements.• Provides a safe, comfortable and therapeutic patient environment.• Initiates and evaluates patient and/or family education.• Documents the patient’s response to nursing and other interventions.• Monitors safety and effectiveness of the environment and equipment.• Provide daily living care to the patient• Supports professional practice by observing the following: Uses knowledge and practices in accordance with 1) the Nurse Practice Act; 2) Standards of Nursing Practice; 3) Standards of Patient Care; 4) Joint Commission, state and local laws; 5) Patient’s Bill of Rights; and 6) NBD, nursing and hospice client policies.• Other duties as assigned.NBD employees are our greatest asset. Job Requirements:
Requirements* Minimum age requirement of 18.* LPN licensed in the state of Ohio. * Graduate of accredited practical nurse or vocational nursing program.* Minimum of two years general nursing experience required. Community health/Hospice or medical/surgical experience preferred. * Compliance with accepted professional standards and practices.* Understanding of Hospice philosophy and needs of terminally ill. * Ability to work in an interdisciplinary setting. * Full command, verbal and written, of the English language.* Satisfactory references from employers and/or professional peers. * Good physical stamina and mental health. Scheduled shifts are typically 12-13 hours.* Successful completion of NBD online application* Successful completion of NBD screening tests with a score of 80% or higher.* Valid driver’s license with maintenance of good driving record and reliable transportation that is insured in accordance with state and/or NBD requirements. * Must be able to travel up to one hour from your home* Valid healthcare CPR certification.* Must present a pre-employment physician’s health certification including a negative TB skin test and/or CXR and other tests as required by NBD policy and/or state law. * Must present a pre-employment fingerprint background check with a “no record” response.
