Christian Care Communities, Adult Day Center located in Lexington, Kentucky is seeking a PRN LPN.

No Weekends - No Holidays - Daytime Hours
Coordinates personal and health related care of participants and conducts participants activities of the Adult Day Center. Conducts all job responsibilities in compliance with all policies and procedures of Christian Care Communities and within the parameters of the Organization's mission, vision, and values.
Performs nursing assessments and care plans, administers and monitors medications, disease screening, health education, rehabilitation therapy and counseling.
Processes intake referrals, assessments of participants and admissions, maintains participants individual charts.
Assists with personal care of participants. This will include toileting, changing soiled clothing, personal hygiene care as needed, assisting in ambulation, cueing clients to independently meet their own needs, monitoring food and fluid intake and all other areas of personal care services.
Maintains participant's records according to regulations and established policies and procedures. Notes and reports any changes in attitude, health, or needs of participants.
Participates in program planning.
Attends educational programs relevant to job responsibilities and in accordance with LPN licensure requirements.
Conducts program activities designed to meet, in accordance with the comprehensive assessment, the interests and physical, mental, and psychosocial well-being of each participant.
Sets up and operates equipment; leads games, singing, discussions and other programmed activities; conducts exercise activities, and interacts with participants on an individual basis.
Works with volunteers assigned to specific projects.
Performs assigned duties to keep the Center safe, clean, and orderly.
Screens and immediately reports suspicious person/information to supervisor or administrator.
Qualifications: Graduate from an accredited LPN Program. Two years experience working with older adults. Must have a current Kentucky LPN license in good standing. Must be first aid and CPR certified. Self-motivated with the ability to mange in crisis situations.
