PRN/On-Call Home Health Licensed Practical Nurse (LPN)
Schedule: Monday-Friday, including on call Weekends

Signature Home Health is a leading provider of Home Health services in the Pacific Northwest. We are looking for a well-rounded LPN to cover the Medford/Ashland Area, providing nursing services to home-bound patients in accordance with established plans of care. The successful candidate will work with a comprehensive team of skilled medical professionals, meeting weekly to strategize treatment plans.
The Home Health LPN will prepare and complete clinical and progress notes the day of service, coordinate medical services, and inform physicians and other medical personnel of changes in the patient's condition/ needs. The LPN will support the education of patients and families and provide comprehensive nursing services enhancing quality of life.

Requirements: The successful candidate will be a skilled LPN, hard working, with excellent customer service skills and a do-whatever-it-takes attitude. Attention to detail is key and the ability to multi-task. The qualified candidate will also be Licensed in the State of Oregon, have a current driver's license, reliable vehicle, and auto insurance. A minimum of two to three years of experience in Home Health, Home Care, and or Long Term Care Geriatric Rehab is preferred. This position requires possible on-call availability weekdays. Ability to work weekends a big plus!
