Resources for Human Development’s Assertive Community Treatment (ACT) Team is currently looking for a full-time LPN. ACT provides supports and services to adults who have a severe and persistent mental illness to assist them in working toward recovery. ACT works with consumers in their environment (home, work, community). ACT utilizes a multidisciplinary team setting including social workers, counselors, vocational specialists, addictions counselors, nurses, psychiatrists, and peer mentors. ACT utilizes a recovery-based, client centered approach to treatment.
Required: *current LPN license *experience with mental illness and/or substance abuse *reliable transportation and insurance
Job Responsibilities include: *assisting consumers with basic needs and ADL improvements *coordination of medical treatment for consumers *educating consumers on medical and medication issues *ensuring medication orders are called/faxed into pharmacy and that consumers receive medication *administering medication (injections) as prescribed by MD *engaging with consumer’s family and natural supports *accompanying MD on rounds *auditing medical charts *completing documentation including notes *working effectively with other team members Location: Jefferson Parish Compensation: competative with excellent benefit package
