The Plaza at Mililani, a senior Assisted Living Facility, is seeking full time, part time and on call Charge Nurses. Summary of Position:
Provides exceptional care for residents under the direction of the Director of Nursing Services and in accordance with facility policies and procedures and state regulations. Responsible for implementing and evaluating resident service/care plans to ensure resident needs are met. Responsible for coordinating and supervising care staff on shift in the absence of Director of Nursing Services. The Director of Nursing Services and Administrator are ultimately responsible for all final hiring and termination decisions. The employee is expected to perform within the scope of his/her license. Reports directly to the Director of Nursing Services regarding nursing or health care issues (reports directly to Administrator if Director of Nursing Services is on vacation or other leave).
Position Requirements:
ï‚· The Employee must retain a current License for Practical Nursing (LPN) or a higher license (Registered Nurse) that is in good standing
ï‚· Acceptable references
ï‚· Maintain required continuing education hours in accordance with regulations governing position
ï‚· Prefer experience in direct care of elderly residents and staff supervision
ï‚· Must meet all employee requirements established in Employee Handbook and Operating Policies and Procedures Manuals
ï‚· Must meet all requirements established by regulation that pertain to employees who work in an Assisted Living setting in accordance with Hawaii Administrative Rules, Department of Health, Title 11, Chapter 90, Assisted Living Facility and regulations that pertain to licensed practical nurses
ï‚· Must have excellent interpersonal skills including ability to work effectively and professionally with residents, family members and other staff members
ï‚· Employee must be self-motivated, well organized and possess excellent care-giving skills
ï‚· Excellent problem identification and resolution skills
ï‚· Team player
ï‚· Ability to attend all required in-services and trainings
ï‚· Excellent ability to provide appropriate documentation and charting as required by policies and procedures and state regulations
ï‚· Genuine interest in the needs of the residents served
ï‚· Ability to provide nursing services as specified by regulation and in accordance with policies and procedures
ï‚· Physical requirements of position: continuous standing/walking/bending/stooping, frequent lifting/pushing/pulling/reaching above shoulders, ability to perform repetitive movements with arms and legs, and occasional sitting; in addition, must
be free of communicable disease and meet community policies and procedures
regarding necessary physical health.
Essential Functions:
ï‚· Provide nursing services under regulatory guidelines including medication administration as specified resident service plans.
ï‚· Ensure care is provided to residents as outlined in individual resident service/care plans.
ï‚· Appropriately document care provided to residents in accordance with policies and procedures.
ï‚· Provide care staff oversight in absence of Director of Nursing Services. Provide training to care aides and participate in evaluations.
ï‚· Know resident rights and provide care in a manner that promotes resident rights and is respectful of resident choices.
ï‚· Maintain confidentiality of all pertinent resident care information.
ï‚· In absence of Director of Nursing Services, as directed by Administrator or Director of Nursing Services, perform functional assessments on residents prior to admission and assess his/her needs for nursing services.
ï‚· Participate in the development and implementation of resident service plans according to community policies and procedures utilizing multi-disciplinary input.
ï‚· Maintain consistent attendance record to ensure proper resident care.
ï‚· Give/receive report at beginning and end of shift.
ï‚· Promptly report concerns regarding resident care and changes in resident condition (mental and/or physical) to Director of Nursing Services (notify Administrator if Director of Nursing Services is on vacation or other leave).
ï‚· Complete Incident Reports as required. Additionally, conduct investigation regarding incident and appropriately documents findings of investigation in resident's health record/chart and on Incident Report. Document preventative plans of action in affected resident's chart (and on Incident Report) to assist in prevention of similar incidents.
ï‚· Oversee the maintenance of the residents' records, investigating and logging all resident related Incident Reports and reviewing and appropriately addressing resident oriented Points of Concern that come up during the shift. Assist in maintaining resident records/charts in proper order.
ï‚· Respond appropriately in emergency situations and facilitate emergency services as needed.
ï‚· Ensure prompt notification and documentation of resident's physician and responsible family member/representative regarding changes in resident's functional health status and immediate notification of resident's physician and representative after an emergency. In addition, promptly notify Director of Nursing Services (notify Administrator if Director of Nursing Services is on vacation or other leave) of resident condition changes in accordance with policies and procedures.
ï‚· Attendance at community administrative staff meetings to stay informed about facility events.
ï‚· Be knowledgeable regarding regulations that govern boarding home facilities and policies and procedures that govern community as outlined in Operating Policies and Procedures Manual. Assist in ensuring care staff follow regulations and policies and procedures.
ï‚· Assist in monitoring and ensuring cleanliness, maintenance and safety of community.
ï‚· Review and audit caregiver documentation and provide documentation of services provided in each resident's record/chart concerning medical needs.
ï‚· Inform Director of Nursing Services of all potential admissions needing additional nursing and all condition changes regarding established residents.
ï‚· Ensure that medication policies and procedures are correctly followed and administer medications if required by resident service plan.
ï‚· Be responsible for proper medication storage, accounting, ordering and proper documentation on the MARS and weekly medication audits in accordance with policies and procedures.
ï‚· Assist Director of Nursing Services in the review of monthly MARS and quarterly physician orders and ensure accuracy of transcription of new orders taken.
ï‚· Collaborate with the Director of Nursing Services, RN (as applicable) and physician regarding residents' health. Ensure orders are appropriately carried out.
ï‚· Assist with making and arranging transportation for health care appointments and services, monitoring resident's modified diets, obtaining and maintaining equipment, and maintaining cleanliness of "clean and soiled" supplies.
ï‚· In coordination with the Administrator, participate in the move-in process regarding new residents and assist in orientation of staff to resident.
ï‚· Assist Director of Nursing Services in processing of care related paperwork corresponding to new admissions.
ï‚· Maintain excellent communication with Director of Nursing Services and care staff to ensure good flow of information regarding resident care.
ï‚· Perform other duties as assigned by supervisor.
Licensing Responsibility
The Charge Nurse is responsible for maintaining current, active license in good standing for duration of employment. The employee is expected to perform within the scope of his/her license.
