LPN - Case Manager (Waltham, Watertown, Belmont)

Come join our innovative nursing team! The Guild for Human Services has an opening for a Licensed Practical Nurse- Case Manager. A residential school for students with developmental disabilities and mental health/behavioral challenges, ages 12 to 22, The Guild operates seven residences in the Waltham area. The LPN- Case Manager works to coordinate health care services for children and young adults who live in our residences. As part of our nursing team, the case manager works with our clinical team, on-site and community based medical services and an array of agencies to provide our students with integrated services and supports.
The schedule for this position is Sunday through Thursday with flexible hours ; this position is also part of an on-call nursing rotation Previous case management experience in a setting support children or adults with special needs is required.
