HOME CARE LICENSED PRACTIAL NURSE (LPN)
DESCRIPTION:
Sholom , one of the premier healthcare and senior services organizations in the Twin Cities, has a PT opening for a skilled, compassionate Home Care Licensed Pracital Nurse (LPN) at its St Paul campus.
The Home Care Licensed Practical Nurse (LPN) provides quality care for clients receiving Home Care services and adheres to professional nursing standards and practices set forth by the Nurse Practice Act.
DUTIES INCLUDE:
• Coordinate overall quality care for clients.
• Assist with new admissions, hospital transfers and discharges as requested.
• Ensure that unlicensed staff duties and responsibilities are accomplished for clients in a timely and professional manner.
• Participate in the completion and review of client care plans.
• Monitor for and address any safety hazards that exist in clients' habits and activities of daily living, and in the building environment.
• Perform supervisory visits as directed by RN Supervisor under Minnesota Home Care rule guidelines.
QUALIFICATIONS INCLUDE:
• Must be licensed as a Licensed Practical Nurse in the State of Minnesota and must maintain licensure.
• Two or more years of experience working with the geriatric population or in a similar care environment preferred.
• Current certification in CPR preferred.
• Valid Minnesota drivers' license with good driving record, proof of valid insurance and have reliable transportation.
• Perform medium work, lifting up to 50 pounds, and occasionally lifting/carrying of objects weighing up to 25 pounds.
• Possess excellent customer service and interpersonal skills.
• Strong computer skills including Microsoft applications such as Word, Excel, Outlook, and applicable clinical software.
