LPN- Intake Coordinator (Tucson)

We are seeking an LPN with either Hospice or Home care background to be our point person for intakes. You will be working with referral sources, transportation Co's, DME, O2 Companies arranging services for new patients.
Must have been an LPN for a min of 2 years
Have an active LPN - AZ License
Good office skills
Computer skills
