Serenity Palliative Care & Hospice, is seeking a motivated, energetic, result-oriented individual to join its team of in-home hospice professionals.
Serenity currently has a full-time Intake/Referral Coordinator position available for a qualified LPN at its fast-paced Tigard Office.
Areas of Responsibility include but are not limited to:

• Assesses referrals from physicians and discharge planners for eligibility and appropriateness of home care.
• Evaluates referrals for appropriateness to hospice services
• Receives and documents verbal and written physician orders (as appropriate)
• Represents agency services to the community.
• Participates in quality improvement activities.
• Assists in maintaining and developing policy and procedures relating to the intake/referral process. * Performs other duties as assigned consistent with skills and training and the goals and mission of the agency.
  Ideal candidate will possess the following:
  Current LPN licensure.
  Graduate of an accredited School of Nursing.
  Successful completion of CPR Certification.
  Minimum of two years nursing experience
  Detail oriented with strong Organizational skills
  Knowledgeable of Medicare guidelines
  Team player
  Effective oral and written communication skills and computer usage skills.
  Data entry with accuracy is necessary with extended time at a computer and on the phone.
