Resident Care Coordinator/LPN (Oro Valley, AZ)

Provides direct supervision of resident care staff in this 34 apt. dementia community. Coordinates training and education of care staff. Schedules and oversees care staff to assure resident safety and care. monitors medication administration. Observes resident health status and takes actions to ensure resident health. Performs ongoing screens of resident physical and psycho-social needs to assure quality proactive care. Maintenance of residents records. Assigns weekly tracking of vitals to rotating shifts. Conducts pre-admission screening of prospective new residents. Communicates with family as needed. Assists with in organizing and developing service plan reviews as needed. Experience with dementia and Alzheimer's preferred.
