Millenium Home Health Agency (TAYLOR) is currently seeking to add a RN and a LPN to our staff. The Skilled Nurse will coordinate care, monitor medical conditions and provide education about medication and disease processes. The oversee the management of illness, obtain lab work as order, provide wound care, manage and instruct regarding ostomies, catheters and other special procedures. They will assist the patient in the area of self care management in order to prevent hospitalizations and return to a pre-episodic level of care(+). The nurse keeps the physician informed of the patient's progress.
The LPN would assist the RN in making sure the the patient's POC is met.
