Comfort Hospice Care is hiring full-time and part-time RN Case Managers and LPN's to join our team. Hospice nursing experience is a plus, but not required; we are willing to train the right person. Enjoy a flexible Monday-Friday schedule with occasional on-call requirements.
Requirements/Responsibilities:
Â• Coordination with other members of the multidisciplinary team to provide high quality care while maintaining HIPAA.
Â• Responsible for making certain that the hospice care provided is compliant with State and Federal regulations (documentation and delivered) through participation in routine chart review and following established process.
Â• Responsible for providing services that are congruent with the policies within the Nurse Practice Act.
Â• Provide excellent customer service to patients and families on a routine basis.
Â• Ability to work autonomously is required.
