MorningStar Senior Living of Applewood is seeking a knowledgeable, compassionate, responsible Licensed Practical Nurse for a part time position. Wonderful assisted living and memory care community. 401k benefits. Rate of pay based on prior experience. Pre-placement physical and drug screen required.
Summary of Position: Day-to-day operations of the community wellness services
Must have current nursing license issued by appropriate state licensing agency.
Experience: A minimum of 1 year experience working with seniors. Basic typing skills along with a working knowledge of personal computing and word processing software are required, preferably in a Microsoft Office environment.
Must exhibit genuine interest in residents; patience and tact in dealing with residents, families, and visitors, as well as care giving and coordination of health care needs within the community. Ability to read and interpret documents such as safety rules, operating and maintenance instructions and procurement manuals. Ability to speak effectively before groups of customers or employees of the company. Ability to effectively communicate in English and understand and follow written and oral instructions.
• Assess health, functional and psycho-social status of residents, initiates individualized service plans, proactively manages care and services for each resident, evaluates effectiveness and maximizes the resident's opportunity to remain in the assisted living environment.
• Performs on-site assessments of residents admitted to hospitals or skilled nursing facilities for treatment, and maintains contact with resident's family with the view to returning to the community.
• Communicate with resident's physician re: residents' status and change of condition.
• Facilitates continuity of care for those residents receiving home health services.
• Provides training, supervision, and monitoring of staff in the administration of medications, to include frequently auditing medications sheets.
• Shares on call duties as required.
