Avamere Rehab of Oregon City is looking for a full-time or part-time experienced Registered Nurse or Licensed Practical Nurse who is clinically driven and self-motivated for this 68 bed skilled nursing facility. A Charge Nurse for Avamere is responsible for excellent patient care in a long-term care skilled nursing clinical setting. You will work with medically complex geriatric patients with various diagnosis such as Alzheimer's, dementia, stroke, brain injury, various neurological disorders, rheumatoid arthritis, osteoarthritis, amputation, joint replacement, fractures, cardiac conditions and pulmonary conditions.
The Charge Nurse follows the Plan of Care established for the patient / resident. In addition, the Charge Nurse supervises throughout their shift the care, treatment and services provided to patient by nursing staff, as well as other healthcare staff assigned to that unit. The Charge Nurse ensures that all documentation, charting and reporting is done correctly and in a timely manor, demonstrate strong ethical and safety practices, and works closely with other nursing staff and attending physician on patient update and/or changes in status.
