Part-time LPN (Depression Care Coordinator) (Blue Cross of Idaho)

POSITION PURPOSE:
To provide guidance to adult members regarding barriers to managing depression-related health conditions. Conduct telephonic patient self-management education including health coaching and consultation. Assist members to change behaviors and to locate and access interpersonal, family and community resources directed at improving target skills that have been lost due to symptoms of their depression.
MINIMUM POSITION REQUIREMENTS:
Required Experience:
- Two (2) years experience in a psychosocial rehabilitation role addressing functional needs of depressed individuals
Required Licenses and/or Certifications: - Licensed Practical Nurse (LPN)
Required Knowledge, Skills and Abilities (KSAs):
Skills: - Medical terminology (including behavioral health)
- Coding (HCPCS, CPT and ICD 9)
- Verbal and written communication
- Microsoft Word – Intermediate
- Microsoft Excel – Intermediate
- Microsoft Access – Intermediate
- Microsoft PowerPoint – Intermediate
Ability to: - Be a team player
- Prioritize and multi-task
- Organize, document and prioritize
- Contribute to program goals and objectives
- Perform telephonic health coaching to members whose target skill have been lost due to their symptoms of depression as evidence through their PHQ-9
PREFERRED QUALIFICATIONS: - Call center knowledge
Position reports to: Manager, Health Promotion & Disease Management
Starting Rate of Pay: Hours: 29 hours/week
