PT LPN - Adult Day Health Care (Yuma)

Part Time, Non-Exempt
PRIMARY FUNCTION
Provide nursing care to the participants and providing other related basic nursing services to the participants. May perform other duties as assigned.
ESSENTIAL FUNCTIONS
• Process and obtain admission orders from the participant's physician as needed
• Participate in the interdisciplinary team
• Assist in performing nursing assessment within 7 days of admission and every 6 months, or as needed
• Assist in the develop of nursing care plan to specify goals of care, type and frequency of care to be provided, and who will be responsible for providing such care
• Responsible implementation and maintenance of medication chart which includes name of medication, strength, dose, frequency, route, side effects and special considerations of each medication ordered on each participant
• Maintain, observe, evaluate, and document participant's response to treatment in the clinical record
• Provide health education in the provision of care and management of participant's illness, and provide appropriate community referrals to participants and caregivers
• Assist participants in taking medications and/or treatments, as ordered by a physician
• Communicate with the physician, as needed, in regards to participant's conditions and arrange for additive services (i.e., PT, OT, ST), if indicated
• Complete accurate and up-to-date charting, and participate in case conferences with other team members in planning appropriate participant care
• Supervise CNAs, Program Aides and volunteers, as directed
• Responsible for recruiting, hiring, training, evaluating, providing constructive performance feedback and separating employees
• May drive agency or personal vehicle on company business
• May perform other duties as assigned
MINIMUM QUALIFICATIONS
• Current Arizona LPN license
• Valid Arizona Driver License, proof of insurance and Arizona Motor Vehicle Division 39 month Motor Vehicle Report
• Ability to obtain health care certificates including TB clearance, CPR, First Aid and Food Handlers Certificate
• Obtain State of Arizona Fingerprint Clearance Card
• Proven computer skills utilizing Microsoft Office Suite
• Proven strong communication skills -- oral and written are clear, concise, and in an organized fashion using appropriate style, grammar and tone
• Proven ability to facilitate an interdisciplinary approach for providing quality service to participants
• Ability to stoop, climb, bend, and reach
DESIRED QUALIFICATIONS
• Bilingual English/Spanish, verbal and written
• Additional years of directly related experience
Part Time, Non-Exempt position.
