Adult Day RN Case Manager
Program: Home and Community Based Services
Direct Supervisor: Executive Director
POSITION SUMMARY
The Adult Day RN Case Manager is responsible for coordinating personal and health related care in compliance with best practices in nursing and conducts activities for participants in the Adult Day Center while maintaining compliance with all applicable laws, regulatory and organizational standards.
Required Education: Associates degree in nursing from a 2-year college or technical school
Required Experience: 2 years' experience working with older adults
Required License and/or Certification: Registered Nurse license and Certification in CPR and First Aid
Business travel is a requirement of position; proof of valid driver's license with a clean driving record
If personal vehicle is used to transport self or others; proof of insurance of $100,000 single limit of liability
Criminal background and drug screening required.
