JOB DESCRIPTION /ESSENTIAL JOB FUNCTIONS
POSITION TITLE: REGISTERED NURSE
POSITION SUMMARY:
The registered nurse is responsible for providing and documenting skilled nursing care in accordance
with care developed for each individual patient in accordance with physician's orders when applicable.
POSITION REQUIREMENTS:
1. Graduate of an accredited school of nursing
2. Currently licensed as a Registered Nurse in the state
3. Complies with accepted professional standards and practice
4. One year prior professional nursing experience, home care experience preferred
5. Current CPR
6. PPD or chest x-ray if applicable
7. Successful completion of Maxim screening examinations with a score of 80% or greater
8. Participation and completion of Maxim's Competency Program
RESPONSIBLE TO: Director of Clinical Services and/or Clinical Supervisor and/or Registered Nurse
PERFORMANCE STANDARDS:
1. Completes an initial assessment of patient and family to determine home care needs
when applicable. Provides a complete physical assessment and history of current and
previous illness (es)
2. Regularly re-evaluates patient nursing needs
3. Initiates the plan of care when directed and makes necessary revisions as patient status
and needs change
4. Uses health assessment data to determine nursing diagnosis
5. Establishes goals based on nursing diagnosis and incorporates therapeutic, preventive,
and rehabilitative nursing actions. Includes the patient and the family in the planning
process
6. Initiates appropriate preventive and rehabilitative nursing procedures. Administers
medications and treatments as prescribed by the physician
7. Provides health care instructions to the patient as appropriate per assessment and plan
of care
8. Identifies discharge planning needs when applicable
9. Adheres to Maxim policy and procedure
10. Attends case conferences, team meetings, education in-services as required
