POSITION: Licensed Practical Nurse PT
STATUS: NON-EXEMPT

QUALIFICATIONS:
• Must relate well to clients with severe-profound disabilities and elders with Alzheimer’s and related dementia. Be able to accept and understand them as they are and work effectively for and with them, even in stressful situations.
• Must have sufficient stamina to spend up to twenty-nine (29) hours per week standing and sufficient strength to assist with lifting and transferring clients, as needed, to care for them.
• Must maintain a current LPN nursing license.
• Must maintain a current CPR certification
• Be able to communicate preferably in English
• Must be able to obtain as Adult Day Care/Food Handler Health Card.
• Must demonstrate the philosophy of the Salvation Army.
JOB RESPONSIBILITIES:
• Develop appropriate Nursing Care Plans for each client with RN--review and update, as needed.
• Complete quarterly and yearly nursing assessments and Care Plan update.
• Audit charts quarterly. Purge charts, as needed.
• Assist with intake and screening prospective clients, via phone referrals and personal visits. Send out necessary papers and give tours, as needed.
• Assist with initial Nursing Assessments with RN review.
• Assist with establishing chart for new clients and post to admit book.
• Administer medications per prescription orders, document on medication sheets, note and report adverse reactions. Update and copy medication administration sheets monthly.
• Receive and obtain meds from family, as needed, ensuring that upon receipt, meds are locked in proper safe storage.
• Maintain narcotic sign-out sheet and count for controlled substance.
• Discuss current care, abilities and needs of clients with family and caregivers, as needed.
• Observe clients for condition changes. Note it in clients' charts and report books, communicate changes to families/caregivers or physicians, if needed.
• Assess and triage in emergency, treat when possible. Call 911 when necessary. Support and prepare clients for transport. Copy and prepare proper papers for EMTs. Notify families/caregivers.
• Schedule, prepare, send and receive yearly assessments. Send out reminders, if needed.
• Assist with daily sign-in sheets, ensuring that each client is signed in and out. Include time, method of transportation and person responsible. Sign off at the end of each day after proper count has been established. Write count at the bottom of the sheet. Accurately transfer the information to the attendance cards.
• Assist with, or complete, weekly Blood Pressure sheet. Prepare new weekly list at the end of the month. Prepare new weekly Blood Pressure sheet at the end of the year.
• Assist with serving meals and feeding clients, as needed. Monitor meals ensuring that special diets are adhered to. Assist with documenting percentage of lunch consumed by each client.
• Assist nursing aides, as needed, with clients. Teach proper techniques and sanitation, as needed.
• Assist with loading and unloading buses, as needed. Be aware of scheduled time for buses. Contact proper agency if bus is late. If transportation is more than thirty (30) minutes late, notify families/caregivers to inform them of same and discuss alternate pick up.
• Comply with nurse's dress code that requires professional attire.
• Maintain confidentiality of clients and the Adult Day Care Program
• Other duties as designated by the ADC Director
REPORTS TO: Adult Day Care Director
PHYSICAL REQUIREMENTS
While performing the duties of this job, the employee is regularly required to walk, stand, climb or balance, stoop, kneel, crouch, crawl and reach with hands and arms. The employee must regularly lift and/or move up to 20 pounds, frequently lift and/or move up to 30 pounds. Specific vision abilities required by this job include close vision, distance vision, color vision, depth perception and ability to adjust focus.
