Reliant Home Health is seeking RN’s and LVN’s and HHAs to join our home health agency. Applicant will be responsible for following physician plans of treatment under the direction of the Director of Patient Care Services. The RN’s are responsible for case management of the client’s plan of treatment and work collaboratively with the LVN and other team members.

HOME HEALTH REQUIREMENTS

• Demonstrated proficiency in OASIS documentation for RN

• Demonstrated phlebotomy and wound care skills for LVN

• At least 1-3 years home health nursing experience

• Excellent communication skills

• California nursing license

• CPR certification

• CA drivers license / car insurance

• Current TB test/ physical

• Must pass criminal background check

• Negative drug screen
